1. Login to Zoho
- Click Sign in using Google
- Click your ViTel Health Email
- Click Patients
- Click Patient's Name
5. Go to CCM Health Conditions and verify that health conditions are present. However, if patient is only doing RPM, verify under RPM Health Conditions.
6. Go to Care Team
- Find or type name of Health Coach in Patient Care Manager
- Click name
- Click check mark
7. Go to Enrollment Note By
- Find or Type your name in Enrollment Note By
- Click on your name
- Click check mark
8. Go to Import/Update Into/DBEhr
- Click Import/Update Into/DBEhr
- Click the check box that appears
- Click check mark
- Wait a minute
- Click reload
- Verify that Import Status says "Success".
- If it doesn't say "Success" nor "Failure", repeat steps 3-5 since it has not updated
- If it says "Failure", go to Fix a failed import in Zoho
- Verify that EHR ID is present
