TABLE OF CONTENTS
A. Login to Zoho
- Click Sign in using Google
2. Click your ViTel Health Email
- Click Patients
- Click Patient's Name
5. Verify if patient is already on Dashboard by checking their tags (as seen in the screenshot below). If patient is not on Dashboard, On Dashboard tag will be absent.
B. Review Information
- Go to CCM Health Conditions and/or RPM Health conditions
- Verify that CCM has at least 2 qualifying conditions and RPM has at least 1 qualifying condition
- Verify CCM and RPM Care Plan Notes are not empty
- Copy and paste health condition to notes if empty
- Review CarePlan Creator is filled
- Go to Patient Care Manager, click Health Coach Name and click checkmark
- Review that Patient Physician is filled
C. Go to Import Settings
- Review that Create CCM Enrollment Note or RPM Enrollment Note is checked
- Go to Enrollment Note By and click your name
- Click check mark
- Go to Import/Update Into/DBEhr and click the check box that appears
- Click check mark
6. Wait 1 minute and click reload
- Verify that Import Status says "Success".
- If it doesn't say "Success" nor "Failure", repeat steps 5-6 since it has not updated
- If it says "Failure", go to Fix a failed import in Zoho
8. Verify that EHR ID is present
- If the patient was not already on the Dashboard before this import, proceed to step 10.
- If the patient was already on the Dashboard prior to this import, go to Create Enrollment Note in Dashboard. Then proceed to step 10.
- Slack Health Coach via direct message to notify them of this new enrollment
Optional: Review in Dashboard
- Login to Dashboard
- Go to Search and type Patient's Name
- Go to Health Coach and remove your name from field by hovering mouse over down arrow and clicking X
4. Click on Patient Name
5. See Dashboard Profile of patient
Optional: Edit Name of Patient in Dashboard
1. Click Edit Patient
2. Edit Name information if needed
3. Click Update Patient
