Find Direct Referral in Athena

A. Login to Athena

  1. Click to open VH Client Details
  2. In VH Client Details, click EHR tab
  1. Go to the row with the Client that you are interested in
  2. Scroll to right until you see Enrollment Team Only and copy username in grey box (select and press ctrl C)
  1. Click to open Athena
  2. Go to username and paste username (press ctrl V)

7. Go to VH Client Details and copy password in black box (select and press ctrl C)

  1. Go to Athena and paste password (press ctrl V)
  2. Click Log In
  3. Optional: Go to VH Client Details and click X to close file

11. Click Go

B. Check for Doctor's Messages

  1. Go to Message Icon in bottom right corner
    1. If the number will be greater than 0, click Message Icon
    2. If the number is 0, there are no new messages. No further action required. Go to Log out of Athena
  2. Conversations will appear on the right side, click on any new messages to see if there are any direct referrals from Doctor.
  3. If there is mention of a direct referral, highlight and copy (ctrl C) patient's first and last name so that you may search up the patient's information.

C. Find Patient Information

Create patient in Zoho

  1. Go to search bar and paste (ctrl V) patient's name
  2. Click magnifying glass icon
  1. Find patient and scroll to the right and click Chart
    1. (If patient doesn't appear in the search, try again with last name only

Note and/or copy (using highlight, then CTRL c) the following information for Zoho patient chart:

  1. Name
  2. Sex
  3. Date of birth
  4. EHR ID for Zoho's Client EHR ID
  5. Hover mouse over telephone number to see additional numbers (mobile and home) and note primary phone
  6. Hover mouse over insurance shown to see primary and secondary insurance names and member IDs

D. Find Patient Diagnosis

There are multiple areas where you can find diagnosis.

Option 1: Note and/or copy (using highlight, then CTRL c) at least 2 Chronic Illnesses listed in Last Visit with Internal Medicine Report

Option 2: Note and/or copy (using highlight, then CTRL c) at least 2 Chronic Illnesses listed under Problems

Option 3: Note and/or copy (using highlight, then CTRL c) at least 2 Chronic Illnesses listed under Problems (Tab) (includes ICD codes)

  1. Click Problems and note the ones of chronic illnesses (these cannot be copied)
  2. Click Problems to close

Option 4: Some Doctors do not list problems in those areas. Instead they list in Visit notes.

  1. Click Visits
  1. Find most recent visit (any visit is fine as long as it's  within the last 6 months)
  2. Click blue arrow beside visit
  3. Click Full Encounter Summary

5. Check if there are any chronic problems listed. If there are none, repeat steps to check any another visit listed within the last 6 months for chronic problems listed.

E. Notify Doctor

  1. Click Message Icon
  2. Click the recent message with the Direct Referral to reply

 

  1. Go to EHR Client Templates
  2. Find client associated with the communication template.
  3. Click New Referral communication template that applies and copy (ctrl C).
  1. Go to Athena
  2. Go to Message and paste
  3. Add initials
  4. Click Send

F. Log out of Athena

1. Click Log Out

2. Click Yes
