1. Login to Zoho and find patient chart
- Login to Dashboard
- Remove Status active
- Remove Health Coach
- Search for the same patient's chart
- Click patient
- Go to Create CCM Care Plan OR Go to Create RPM Care Plan
Add CCM Care Plan to Patient in Dashboard
1. Click Create CCM
- Go to Health condition, input at least 2 chronic conditions from Zoho
- Go to Select Template and click CCM Care Plan
- Check if Patient Information has populated. If it has not, input information like Patient Name, Date of Birth, Age, Sex, and Clinician Name
- Click Create CCM
Add RPM Care Plan to Patient in Dashboard
1. Click Create RPM
- Go to Health condition, input at least 1 chronic condition from Zoho
- Go to Select Template and click RPM Care Plan
- Check if Patient Information has populated. If it has not, input information like Patient Name, Date of Birth, Age, Sex, and Clinician Name
5. Click Create RPM
