Create Outreach Log in Zoho as an Enrollment Specialist

This article explains how to create and fill in the details for a new outreach log in Zoho according to different outcomes.

A. Login to Zoho

  1. Go to Zoho
  2. Click Sign In

3. Click Google

4. Click your account

5. Click Patients

6. Click Patient's Name

B. Create New Outreach Log

  1. Click Outreach Logs

2. Click New

3. Enter details into Outreach Log as shown below

Note: Blue asterisk indicate optional fields depending on outcome of the call

Go to step 4, for instructions for Outcome

4. Outcomes

Go to Outcome and go to steps corresponding to your outcome

  1. Voicemail
  2. Unable to Leave Voicemail
  3. Enrolled
  4. Declined
  5. Spoke to
  6. Other

a. Voicemail

  1. Click Voicemail
  1. If this is not the last outreach, go to step 3. If this is the last outreach (usually outreach number 3), go to Move to FTO status and click box
  2. Click Save

b. Unable to Leave Voicemail

  1. Click  Unable to Leave Voicemail

2. Go to  Outreach Notes and type any relevant additional information

  1. If this is not the last outreach for the patient, skip to next step.
    1. If this is the last outreach for the patient, move patient to FTO by clicking the box next to Move to FTO status.
  2. Click Save

c. Enrolled

  1. Go to Outcome and click outcome Enrolled
  1. Go to Outreach Notes and type:
    • Appointment date/time (patient's time zone) with name of assigned health coach
    • patient confirming their identity
    • patient verbally agreeing (“Yes”) to enroll in the program
    • acknowledgment that the patient understands the program is fully covered and/or any amount they are responsible for
    • confirmation that the Health Coach’s name and contact information was provided (via phone, email, SMS, etc.).
    • any other relevant information the Health Coach should be aware of
  1. Go to Appt w/HC and put the date and time of the first appointment.
  2. If patient will do the standard Welcome appointment without devices, skip this step and go to step 5. If patient is enrolled in RPM right away, go to RPM Care Plan/Note, and click the relevant option.
  3. Click Save
d. Declined
  1. Click outcome Declined

2. Go to Declined Reason and click applicable reason

3. Go to Outreach Notes  and type add any relevant additional information

4. Click Save

e. Spoke to

  1. Go to Outcome and click Spoke to - need call back, Spoke to - check insurance, or Spoke to - call back 2 months

2. Go to Call Back Reason and click relevant reason

3. Go to Outreach Notes and type any relevant additional information

4. Click Save

f. Other

  1. Click FU after enrollment, Other read notes, Pt Deceased or Already Enrolled

2. Go to Outreach Notes and type any relevant additional information

3. Click Save

C. Edit Outreach Log in Zoho

1. From Patient's Zoho Chart, click Outreach Logs in the top left corner.

2. Hover mouse over the Outreach log that you would like to edit and click pencil that appears

  1. Click whichever field you would like to edit and make necessary changes
  2. Click Save

5. You should see the newly edited Outreach Log under the Patient's profile
