When you use Import/Update Into/DBEHr after patient is already in Dashboard, only the patient's information (name, address, date of birth, telephone number, etc.) is updated. The enrollment note must be manually added to Dashboard.
TABLE OF CONTENTS
A. Login to Zoho
- Click Sign in using Google
2. Click your ViTel Health Email
- Click Patients
4. Go to Search records and type patient's name
5. Click on patient name
- Copy patient name (highlight and Ctrl c)
- Click Outreach Logs
8. Hover mouse over the Enrolled Outreach Log and click pencil icon
9. See the Outreach note and keep Zoho open as you go to next steps
B. Login to Dashboard
- Click link to go to Dashboard
- Click Continue with Google
3. Click your google account
4. Click RPM/CCM app
5. Click View Data Management System
- In Dashboard, go to Health Coach and if your name is present, hover mouse over down arrow and click X.
- In Status, click X to unselect active
- Paste patient name into search bar
- Click on patient name
C. Add Enrollment Note in Dashboard
- Click the start/time button
- Go to Description and type CCM Onboard
- Click Start Task
- Go to Select Template and type onboard
- Select @CCM-Initial-Onboard
6. Go to Zoho, and copy enrollment note (highlight and ctrl C)
- Go to Dashboard, in Note paste at the bottom of the template (ctrl V)
- Go to Duration in minutes and enter 20
- Click End Task
