Unenroll patient in RPM/CCM as a Health Coach

  1. Inform patient
    • Due to unenrollment, their equipment will be unlinked to our system and we will no longer see or monitor their vitals.
    • We will send them a prepaid label to return the equipment
      • Ask if we can send it via email and confirm email address.
        • If no email
          • and allow texting, confirm number and share Annabelle will text patient.
          • nor can text, share with Annabelle will call patient.
  2. Unlink/Deactivate Devices through the Dashboard
  3. Request for label for Return RPM Equipment (New or Used)
  4. Place note in dashboard (template in dashboard- .Unenrolled RPM )

5.    Create Comment "Unenroll in RPM on date"

6.    Edit Care Plan(s) and add end date either as Today or the date unenrolled (to change active care plans to inactive. Be sure to do both RPM and CCM if unenrolling in both)

7.    Edit Patient and change status to "Inactive"

8.    Inform PCP (copy note from dashboard to client's EHR)

             Sample below for TMG in ECW

Add patient to Inactive Patients Log (Temporary to be sure it shows on billing reports)
(You should have changed the status to inactive AND inactive the care plans)
