1. Go to https://access.vitelhealth.com/ and login by Continue with Google using your ViTel Health Email.

2. Go to RPM/CCM Remote Patient Monitoring

3. Click View Data Management System

4. It will automatically bring you to Patients. Wait 10 seconds for it to load (as shown) to view your patients.

5. Search Patient
6. Open up patient Chart

7. Click Devices
8. Copy gateway ID and paste in comment

9. Click Deactivate for each device patient is no longer using. (All devices if un-enrolling in RPM)

All devices should be removed from Devices List
