1. Place note in dashboard (template in dashboard- .Unenrolled CCM )

2.    Create Comment "Unenroll in CCM on date"

3.     Inform PCP (copy note from dashboard)
             Sample below for TMG in ECW

4.     Add patient to Inactive Patients Log
